Principal Duties:

1. Assist in the planning, implementation and evaluation of interventions to meet the identified needs of the service users receiving care and support.

2. Involve service users in the development of their care and support packages, enabling them to make choices whilst ensuring as much independence as possible to do things for themselves.

3. To assist in the assessment of risk and the development of guidelines which mange that risk for the service user within the least restrictive framework possible.

4. Assist in monitoring of health and behaviour of service users reporting changes to the shift leader.

5. Participate in service user meeting preparing reports and presenting information about your service user to other professionals as necessary.

6. Help to develop and maintain friendships and relationships between the service user and others (including relatives) internal and external to the home, ensuring that you act professionally and maintain confidentiality.

7. Act as a key worker to named service users ensuring that they have a Person Centre Plan which is implemented reflects the lifestyle and choices of the service user.

8. Help to support and enable service users to understand and meet their emotional, racial, cultural, and religious needs and preferences whilst maintaining privacy, dignity, and respect at all times.

9. Involve and support the service user in undertaking household tasks including, shopping, cooking, and cleaning.

10. Being an active member of the multi-disciplinary team in assisting in organised activities for clients; helping to create a homely atmosphere for patients and monitoring clients personal hygiene standards and ensuring these are maintained
